How Do I....
Obtain A Repeat Prescription?
- These are issued at the doctor’s discretion and are normally for patients on long-term treatment.
- Please use your REPEAT MEDICATION REQUEST FORM which is attached to the computerised prescription.
- Please request only the items needed and do not hoard medicines at home as this is both unsafe
and wasteful.
- Please advise your GP if, for any reason, you are no longer taking any medication listed on your
repeat form.
- The form may be posted (enclose an SAE if you wish us to post it back),
handed in, faxed to 01784 464360 or emailed to shgscripts@nhs.net. Repeat prescriptions
can also be requested via this website, if you are registered with us to use
the practice internet services. To register, enquire at reception.
- Allow two working days before collecting your prescription.
- We cannot take requests over the phone as this could lead to serious errors and also blocks the
telephone for urgent calls.
- Arrangements can be made for your prescription to be collected by the local pharmacy so that you do
not need to call back to the surgery - ask at reception.
